All samples should be submitted in the proper conditions, accompanied by written instructions on a properly completed Chain of Custody form with the following minimum
information:
- Company name, address, phone number and person receiving the report.
- Type of samples.
- Date and time of sampling.
- Name of sampler.
- Analyses needed on each sample.
- Project name or sample site.
- Billing information (P.O. number, address, etc.).
Also, any other additional information such as ranges of expected results or description of the sampling site or any hazardous characteristics of the samples is very useful for expeditious testing and interpretation of results.
